Ten Things Your Family Should Know About Your Loved One
If the Loved One is slowly passing from a disease or injury, he or she may still be able to put their Estate Plan in proper order. If a Will, Trust, Power of Attorney, or Health Care Directive has not yet been made, it can still be done as long as the person is mentally competent to do so.
I recommend a Personal Representative be designated by the Loved One or by the family to attend to their final affairs.

- Date and place of birth.
- Social security number.
- Children from all marriages and other relationships
- Location of the Will or Trust documents
- Location of the safe deposit box
- Location and account numbers of all bank accounts, certificates of deposit, etc.
- Location of all insurance policies and policy numbers (including fraternal and paid-up policies)
- Location and account numbers of stocks, bonds and other securities
- Location of all real estate and Property Identification (PID) Numbers
- Veteran’s identification numbers (if any)
Armed with this information, the Personal Representative can get the Estate matters concluded with the greatest efficiency and least family disruption.
